At Doors of Hope, each consumer receives an initial Diagnostic Assessment performed by a member of our clinical team made up of qualified practitioners. Assessments identify the presenting problem, that is what the person’s issues are that brought them to treatment, inherent risks and safety issues, diagnosis as described by their symptoms and the resulting functional impairments. Our person-centered assessments incorporate consumer strengths, stage of change and cultural factors that impact treatment. The assessments also include discharge and transition summary.
Doors of Hope professional team of CSWs seeks to identify and coordinate resources within the community to assist consumers to recover and be rehabilitated to the community. While under the overall supervision of the Clinical Director, the CSWs report directly to the Clinical Manager. CWSs are divided into two teams each led by a Team leader supervising the efficient delivery of services and effective documentation of these. Each CSW shall manage a caseload of maximum 20 consumers.
Doors of Hope has a team of therapists who provide counseling services: - individual, family and group face-to-face and telehealth services for symptom and behavior management; development, restoration or enhancement of adaptive behaviors and skills; and enhancement or maintenance
of daily living skills.
Our licensed professionals provide medication–somatic services monitoring the side effects and interactions of medications and the adverse reactions a consumer may experience, and provide education and direction for symptom and medication
self-management.
